Mosaic Psychological Services, Dr. Kimberly Anzek works with children ages 3 and older, and provides Psychological evaluations, Autism testing, Educational and Dyslexia testing, ADHD (attention deficit hyperactivity disorder) Testing.  

Insurance Accepted: CareSource, Medical Mutual, Paneled UHC (United HealthCare), Tricare, Anthem, Aetna and Cigna.

Ms. Karen Fowler Specializes in ADHD, anger management, asperger's syndrome, behavioral issues, borderline personality, coping skills, development disorders, family conflict, oppositional defiance, peer relationships, self-harming, suicidal ideation, academic underachievement, anxiety, autism, bipolar disorder, depression, emotional disturbance, grief, OCD, parenting, self esteem, and sexual abuse. She accepts EAP (Employee Assistance Programs), aetna, beacon health, cigna, empathia, guidance resource/compsych, impact solutions, matrix, magellan, military one source, morneau shepell, new directions, ohio health, optum, workplace options, and R3 continuum. They provide psychological assessments, neurofeedback, and therapy; autism, ADHD, psych evals for mood disorders, learning disabilities, cognitive issues, and neuropsych evals. They accept most insurances (including Medicaids). 

Payments: Families are provided with a detailed invoice and the agency accepts HSA payments.
