preparing for your appointment
If you have an upcoming appointment with our clinical nutrition team, please follow these steps to make sure your appointment is as helpful as possible:
1. If your child is younger than 1 year of age, please complete the following:
Infant nutrition history online form submission
Or print this Infant nutrition history form and bring it to your appointment
2. If your child is older than 1 year of age, please complete the following:
Food frequency and healthy habits online questionnaire submission
Or print these forms
3. Arrive at least 15 minutes before your appointment to complete check-in. Bring the completed form(s) with you if you did not fill out the online submission. A parent/legal guardian must attend the appointment to provide consent and medical history. You will also be part of the discussions about your child's treatment.
4. Plan for your appointment to take 30 to 60 minutes. If you arrive late for your appointment, the duration of the appointment may be shortened. If you arrive later than 15 minutes past your appointment time, there is a chance your appointment will need to be rescheduled.
