Your child’s success is important to us and dependent on your commitment to consistent attendance and participation with the home program. To help us provide safe, quality care, families must agree to the following:
- Attend therapy on time and regularly. You are expected to attend and be on time for at least 80% of your scheduled visits. If you are late, we will not be able to see you for the full visit as we are usually fully booked.
- If you need to miss an appointment, please let us know in advance. If changing to a video visit is an option for your child, you can request this when you call. We have limited ability to reschedule your visit later as therapy schedules are usually fully booked.
to contact your therapist:
If you have questions for your therapist, it is best to ask those during the therapy session. If you need to reach them outside of a therapy session, send them a message through MyKidsChart or call 937-641-5000 and enter your therapist’s extension or request to be transferred to their extension. If you need to know your therapist’s name so you can leave them a message, this can be found on the text reminder or in MyKidsChart.
