For all visits, please wear comfortable and loose clothing and bring:
- Your insurance information and photo ID
- Your child’s labs/bloodwork or imaging results (if you have it). You may send this by fax before your appointment to allow time for review. 937-641-5484.
- A list of questions
- Any required school or work forms.

The appointment will be approximately an hour so the provider may develop a personalized treatment plan and answer all of your questions.
