Dayton Children’s provides a least restrictive alternative environment for patients 11 through 17 years of age with psychiatric needs, who meet the following criteria:

• Patient is at risk of seriously injuring themselves or someone else
• Patient is deemed medically clear by an emergency department physician, attending or admitting psychiatrist
• Patient is able to participate in and benefit from therapeutic activities on the unit
• Parent and/or guardian must be able to participate in family therapy and in the discharge conference
• There must be a bed available and appropriate staffing on the unit

The inpatient unit is not for patients with a primary diagnosis of a substance abuse disorder, detoxification for substance abuse, eating disorders, autism spectrum disorder or intellectual disability.

If a patient has attempted suicide or is otherwise not medically stable, the patient should be brought to:

• Dayton Children’s emergency department

If a patient is medically stable and coming from home or a care provider’s office:

• The patient should be sent to the behavioral crisis center at Dayton Children’s to be evaluated by a clinician
• If the patient meets criteria for inpatient admission, they will be medically cleared by an emergency department physician
• If a bed is available on the unit, the patient will be admitted
• If a bed is not available, crisis center staff will search for an available bed in another facility

If a patient is coming from another hospital’s emergency department:

• That hospital’s crisis worker can contact the behavioral crisis center at Dayton Children’s to request direct admission to the inpatient unit (937-641-4480)
• All admission criteria must be met for direct admission

Connection to community resources and schools will be done prior to discharge.

If a parent/guardian signs the release of information form and provides the name of their primary care physician, a discharge summary will be sent to the primary care physician within 30 days of discharge.

Children and adolescents receive medical and psychosocial evaluations by child advocacy whenever there is a concern a child may have been abused or neglected.

The clinical team is comprised of physicians, nurse practitioners, social workers, forensic nurses and a medical assistant. Team members have specialized education, training and experience in evaluating concerns for child maltreatment.

• Outpatient clinic evaluations (initial assessments and follow-up)
  • Initial evaluations include identifying injuries or illnesses, assessing overall physical and mental health, rendering medical opinions for any findings identified, and providing referrals and resources for identified needs.
• Consultation for patients admitted to Dayton Children’s inpatient and emergency departments
• Phone consultation for patients of community providers
• Education/training on child maltreatment topics, such as:
  • Triage and management of suspected sexual abuse
  • Recognition and evaluation of suspected child physical abuse
  • Medical neglect
  • Mandated reporting responsibilities
  • Medical child abuse (formerly Munchausen Syndrome by Proxy)

• Forensic interviews — these are typically conducted by children’s advocacy center staff or investigators when indicated.
• Behavioral health counseling services — please refer to your local children’s advocacy center or Dayton Children’s Mental Health Resource Connection at 937-641-4780.

The chronic pain management clinic at Dayton Children’s provides outpatient care for patients up to 22 years of age who are experiencing chronic pain conditions, including:

• Abdominal pain
• Back, chest and extremity pain
• Complex regional pain syndrome (reflex sympathetic dystrophy)
• Fibromyalgia
• Pain associated with Ehlers-Danlos syndrome, juvenile rheumatoid arthritis, sickle cell disease and other conditions
• Nerve pain caused by an injury
• Post-surgery and trauma pain
• Oncology and sickle cell pain

Chronic pain management clinic is not able to provide services for:

• Headaches and migraines – please refer to neurology
• Schizophrenia, bipolar disorder, personality disorders or any significant co-morbid psychiatric diagnoses – please refer to psychology
• Patients over 22 years old

Developmental Pediatrics provides services for patients under 5 years of age with concern regarding:

• Developmental milestones in communication, physical skills, learning skills and/or social skills
• Developmental services for children with Down syndrome or other genetic disorders
• Our team will assist the family in coordination of community services
• Autism Diagnostic Clinic (up to patient’s 4th birthday)
  • Diagnostic screening
  • Community resources including social work
• Complex ADHD (5 to 16 years of age)
• Newborn Follow-Up Clinic

Nearly 90 percent of our surgeries are outpatient, so kids are discharged the same day, allowing them to rest and heal in the comfort of home. Our pediatric surgery team provides exceptional follow-up care in our surgery clinic after patients are discharged home.

Surgery clinic provides pre-surgery consultations and post-operative care. A typical visit to the surgery clinic involves a physical exam, conversation about any test results and medication needs, and time to answer questions and talk about the potential need for surgery with a pediatric surgeon.

Pediatric sleep medicine at Dayton Children’s treats patients for a variety of conditions, including;

• Sleep apnea
• Insomnia
• Periodic limb movement disorder
• Restless leg syndrome
• Narcolepsy and other disorders of excessive daytime sleepiness
• Sleep onset association disorders
• Behavioral sleep disorders
• Parasomnias and other unusual activities during sleep

The sleep lab is where testing for sleep disorders occurs. Our American Academy of Sleep Medicine accredited sleep laboratories are all equipped with state-of-the-art polysomnography equipment and all studies are interpreted by board certified sleep physicians. Our sleep center is dedicated to caring for babies, children, teens and pediatric patients with special needs. We have designed the lab so that an adult family member has a place to stay with their child during the overnight sleep test.

All children with suspected sleep disorders should be evaluated in sleep clinic before undergoing testing in sleep laboratory.

• To ensure that the most appropriate study is ordered based on patient history and examination
• Patient will be evaluated by board certified physicians in pediatric sleep medicine
• Patient will be adequately prepated by receiving education from trained sleep nurses and respiratory therapists
• The patient and family will have an opportunity to visit the sleep lab prior to the study
• A desensitization visit in the sleep lab may be aranged for specific patients such as; PAP titrations or children identified at high risk for not tolerating a complete sleep study
• Dayton Children’s staff will obtain insurance sleep study prior authorization following the sleep clinic visit

• Overnight diagnostic sleep study to evaluate for sleep apena and periodic limb movements
• Overnight Titration sleep study to determine optimum therapy
• Overnight extend EEG sleep study to evaluate abnormal behaviors during sleep
• Diagnostic or titration sleep studies for infants during daytime
• Daytime tests for patients with excessive sleepiness
• Actigraphy to evaluate sleep duration and circadian rhythm

Patients referred from pediatric ENT and pediatric pulmonologists are able to be directly referred to the sleep lab when this exception is met: repeat diagnostic study after surgical intervention.

• Children will be studied at their baseline
• Studies will not be performed if the patient is febrile, ill with viral symptoms, on new pain medications or after recent surgery
• Results will be delivered to the ordering clinician and to the patient’s primary physician of the study via EPIC or fax
• In case of urgent findings, the sleep lab physicians may contact the referring physician with results
• If severe sleep apnea is diagnosed, the sleep lab physician will admit to Dayton Children’s and consult our subspecialists or arrange home therapy if indicated
• The sleep lab physicians are available to discuss results by calling the sleep center at 937-641-5004

If you suspect your patient has a hemangioma, vascular tumor or a vascular malformation, the vascular anomalies clinic team can help. Reasons for referral may include:

• High-risk location such as head, face, neck, spine, perineal area, lower back
• Multiple hemangiomas (>5 in any location)
• Functional impairment (obstructing eye, involving ear canal, interfering with feeding, use of hands, walking, etc.)
• Recurrent or prolonged bleeding
• Ulceration
• Infections of malformation
• Pain
• Rapidly growing lesion
• Any overgrowth of extremity or one side of the body
• Any blue or purple lesion present at birth (less likely an infantile hemangioma)
•  Vascular anomaly associated with other congenital defects or medical problems
• Reassurance and education of family
• Unsure of diagnosis
• Poor response to treatment such as propranolol
