Have questions about using MyChart for your child’s care? Explore these frequently asked questions to learn how MyChart can help you manage your child’s health online.

A: Patients who wish to participate will be issued a MyChart activation link during their clinic visit or by filling out the online enrollment form. This link will enable you to login and create your own user ID and password.

You can sign up for MyChart in one of two ways:

  • During your child’s clinic visit, you’ll receive an activation link to create your account. Be sure to complete your activation within 24 hours.
  • Online, by completing the MyKidsChart Enrollment Form. This form allows you to request access and create your own user ID and password.

Once your account is set up, you can log in anytime to view your child’s health information. Access depends on your child’s age. To request access, fill out the Online Enrollment Form. For more questions, contact our MyChart Patient Support Line at 1-877-355-3155.

A: Call our MyChart Patient Support Line at 1-877-355-3155 for help. This support line can help when your access link isn’t working, you can’t see your billing information, help deleting or re-activating your account, help with two-step verification and more.

Yes. We take great care to ensure your child’s health information is kept private and secure. Access to information is controlled through secure access links, personal ID’s, and passwords. Each person controls their password, and the account cannot be accessed without that password. MyChart uses the latest encryption technology with no caching to automatically encrypt your session. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

To protect your child’s health information, MyChart uses two-factor authentication (2FA). This adds a second layer of security by requiring a one-time code, in addition to your password, when logging in.

It can take up to 15 minutes to receive your code. For email, check your spam or junk folder and make sure donotReply@childrensdayton.org is on your allowed email list. For text, check to make sure you don’t have 937-230-8588 blocked. If you still don’t receive a code, contact the MyChart support line at 1-877-355-3155.

upload your photo ID

As part of your enrollment process, please upload an image of your driver’s license or a valid photo ID to the form below (important: please return to the MyKidsChart form after uploading to complete your enrollment)

Our care team will validate your identity within 72 hours and complete your MyKidsChart registration. After our team completes your registration, your photo ID will be stored securely in our medical records system. It will not be stored on our website. If you have any questions or concerns, feel free to send us a message or call 937-641-3395.
